DESCRIPTION
The following formats are used to enter data within the CMBHS application:
Checkboxes

How to use Check boxes:
- CLICK inside the box to check;
- A green dot will appear in the circle indicating that you have successfully selected the item
- To remove the check, CLICK inside the same box again
Drop-down Lists
To use drop-down windows:
- CLICK on the arrow
on the right to show the selection list
- If needed, use the scroll bar on the right to show all the items
- CLICK ON the item you wish to choose
- The item will change color to show that you have successfully picked the item
CAUTION
- When using drop down lists, as long as an answer choice is still highlighted, it can accidentally change.
- This is especially true if you use a mouse with a scroll wheel.
- To make sure that your answer does not accidentally change, you must CLICK in the next answer box or anywhere on the CMBHS page to “set” your answer.
Drop-down Selection “Other”
When selecting “Other” as an answer choice in CMBHS:
- In some drop down lists “Other” is provided as an answer choice.
- Most of the time, CMBHS requires you to provide additional information to explain why you selected Other as an answer choice.
- Whenever Other is picked, a textbox will appear.This is where you must TYPE IN the additional information.
- If you do not put the required additional information into the text box and you CLICK ON the Save button, you will get an error message telling you to provide the required information before attempting to save the document.
Multi-select List Box

How to use multi-select list boxes:
- The multi-select list works similarly to the drop-down list except that you can pick more than one answer choice.
- CLICK ON the arrow
to the right of the box to show the list of answer choices.
- CLICK ON the first item you wish to choose.
- To pick the next item, hold down the Control Key (CTRL) on your keyboard while you CLICK ON the other items you want to pick.
- CTRL CLICK ON any item to deselect the item.
Information Tables
How to use Information tables(Client-side Grid):
- CMBHS includes Information Tables when there are several data elements to be documented about several different people or situations.
- It is critical that you remember to CLICK ON the ADD button after entering the required data into the fields.
- Some tables display a checkbox at the top of the table labeled “Show All Columns”.
- If you CLICK IN this box, additional columns of information display to your right.
- You will need to scroll to the right to view the additional columns.
To Add new items to information tables.
- Enter information in the data fields.
- CLICK ON ADD to add the data to the table.
- CLICK ON “Save” to save data to CMBHS.
To Edit items in information tables:
- CLICK ON an item in a row to pick the row.
- CLICK ON the Edit button at the top of the page.
- Edit the information in the data fields.
- CLICK ON ADD.
- CLICK ON “Save” to save data to CMBHS.
To Remove/Restore items in Information Tables:
- CLICK ON an item in a row.
- CLICK ON the Remove (or if already removed, CLICK ON the Restore) button).
- The row will appear with a strike through (or the strike through will be removed).
- CLICK ON Save to remove/restore the row from/to the table.
- CLICK ON Save to save data to CMBHS.
CAUTION:
- If you do not CLICK ON the ADD button after you fill the data fields for the information tables, it will not be saved, even when you CLICK ON the Save button.
- When using information tables, always ADD first, then Save!!
Note:
- You can only restore items before they are Saved to CMBHS.
- After you CLICK ON Save, even if the document is in Draft status, it can not be restored.
Radio Buttons
- CLICK inside the circle that goes with the answer you choose;
- To change your choice, CLICK inside another circle
Note:
- Once you CLICK in a circle and have made a choice, you may change your choice, but you can no longer leave the item blank.
- You must pick one of the answer choices.
Text box
- Most text boxes in CMBHS will accept numbers and letters. If only one kind of character (numbers or letters) are accepted in a textbox, it will be clearly indicated next to the textbox.
- When using a textbox, look for information that indicates the number of characters that you may type into the text box and adjust your documentation to fit the limit.
- If you type in too many characters, the system will stop displaying the characters you type and you will get an error message when you try to save your documentation by CLICKING ON the SAVE button.
Date and Time Textboxes

Errors Documenting the Date in CMBHS
- If you do not type in enough numbers or you type in too many numbers or if you accidentally put in a letter, you will get an error message.
- If you type in a date that is before or after the correct date, you will receive an error message.
- You can not save a document in Closed Complete or Ready for Review until all errors are corrected.
Transfer List Boxes/Transfer Symbols
How to use Transfer List Boxes
- To select multiple items that are listed one after the other, CLICK ON the first item and SHIFT - CLICK on the last item in the series.
- To select multiple items that are scattered throughout the list, CTRL CLICK ON each item.
- Type the first letter of the item you wish to pick, to scroll down to it quickly.
Transfer Symbols

- CLICKING ON this symbol moves the items you have picked from the list of Available Items, to the box on the right, making it a Selected Item.

- Moves items from Selected Items box on the right to the Available Options box on the left.

- Moves all items from the Available Options on the left to Selected Options on the right.

- Moves all items from Selected Options on the right to Available Options on the left.
